STEP 1:ParTEA DETAILS - complete and submit the following form to hold your ParTEA date (you may also email/call with the information). This form does NOT guarantee your date until steps 2 & 3 are complete (see below).

STEP 2: ParTEA AGREEMENT - we will email (or mail) the agreement which must be signed and either faxed to (623) 434-5552 OR mailed/brought to 8340 W. Vogel Ave. Peoria, AZ, 85345.

STEP 3: ParTEA DEPOSIT - a $100 non-refundable deposit is the final step to secure your date. We accept cash, check, money order, or credit card payment (using Paypal).
